I understand that although BOOST FitClub, LLC, equipment, services and programs are designed to provide a safe level of beneficial exercise and enjoyment, there is an inherent risk that use of such facilities, equipment, services and programs may result in personal injury.

I hereby agree to specifically assume all risk of injury to me or my guest(s) while using any of the club’s facilities, equipment, services or programs. I hereby waive any and all claims or actions I may have against the BOOST FitClub, LLC, or its owners and employees as a result of such injury.

The risks include, but are not limited to:
1. Injuries arising from my use of any exercise equipment or machines.

2. Injuries arising from my participation in supervised or unsupervised activities and programs in the exercise rooms, program activities outside the premises, or on adventure trips.

3. Injuries or medical disorders resulting from exercising at BOOST FitClub, LLC including heart attacks, strokes, heat stress, sprains, broken bones and torn muscles or ligaments.

4. Accidental injuries within the facilities, including the locker rooms, showers and dressing rooms.

I acknowledge that I have received, read and understand the rules, and agree to the rules and regulations of this club. I also acknowledge the existence of and the need for rules including those governing the use of BOOST FitClub, LLC equipment and participation in any programs or services. I hereby agree to these rules and regulations and to any future amendments BOOST FitClub, LLC defines appropriate.
