In checking the box below I hereby agree to the following:

1. I understand and acknowledge that participation in yoga classes involves inherent risks, including but not limited to risks from strenuous physical exertion, the use of equipment, and potential contact with other participants, which risks may result in serious physical injury. I voluntarily assume such risks and assume full responsibility for all risks, known or unknown, that arise from my participation in yoga classes at Yoga of Los Altos.

2. I understand that it is my responsibility to consult with a physician prior to and
regarding my participation in yoga. I certify that I am physically fit and well and have no medical condition, illness, or injury that would prevent my full participation in yoga classes or make my participation medically inadvisable or unsafe for me or for others. I acknowledge that it is my responsibility to inform the instructor when I begin a class of any injury or other condition that might affect my ability to participate, and it is also my responsibility to refrain from any specific activity, pose or position that is medically inadvisable or unsafe for me. I agree to inform the instructor immediately if an injury occurs during class.

3. I agree that Yoga of Los Altos is in no way responsible for the safekeeping of my personal belongings while I am at the studio, and I assume the risk of any loss or damage to personal belongings that I bring with me to the studio.

4. In consideration for my participation, I release, waive, and discharge any and all claims against Healing Cultures, Inc., doing business as Yoga of Los Altos, and its directors, officers, employees, and agents (“Released Parties”) for any liability, loss, injury, damages, or expenses arising out of my presence at the Yoga of Los Altos studio or arising out of my participation in Yoga of Los Altos classes, regardless of cause and even if caused by negligence. I agree not to sue any of the Released Parties on the basis of these waived and released claims. I waive the protections of Section 1542 of the California Civil Code, which provides that a general release does not extend to certain claims not known to me at the time I signed this waiver and release.

5. I understand that instructors may come physically adjust me for proper alignment or to help me feel the pose better. If I feel uncomfortable, it is my responsibility to let the instructor know that I do not want to be touched.

6. In the event that the waiver needs to be produced, a copy of the original will suffice.

7. I have read this waiver and understand its terms. I understand that by agreeing to this waiver, I am giving up legal rights and remedies in consideration of my participation in Yoga of Los Altos classes, and I agree to an execute this waiver and release knowingly and voluntarily.

8. If I am executing this waiver on behalf of a person who is under eighteen years of age or otherwise lacks the capability to execute this agreement for themselves, I represent and certify that I am the parent or legal guardian of the person identified above and that I am knowingly and voluntarily agreeing to this release and waiver on such person’s behalf.
