LIABILITY WAIVER AND RELEASE AGREEMENT

THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS – PLEASE READ CAREFULLY BEFORE SIGNING
This Release and Waiver of Liability (the "Agreement") is executed by the undersigned participant ("Participant") in favor of RF Dance Center, its officers, directors, employees, agents, independent contractors, and volunteers (collectively, "RFDC").
1. ACTIVITIES: The Participant acknowledges that they will be participating in dance classes, workshops, and other related activities (collectively, the "Activities") at RFDC. These Activities may include, but are not limited to, ballet, Salsa, Bachata, Cumbia, and Banda dance.
2. RISK ACKNOWLEDGEMENT: The Participant understands that the Activities involve inherent risks, including the risk of physical injury, disability, or death. The Participant willingly and voluntarily assumes full responsibility for all risks, both known and unknown, even if arising from the negligence of RFDC.
3. FACILITY INSPECTION: The Participant acknowledges that RFDC has inspected the facility for safety. The Participant agrees to inspect the facilities and equipment to be used, and if they believe anything is unsafe, it is their responsibility to immediately notify the proper authority at RFDC and refuse to participate further.
4. RELEASE: In consideration of being allowed to participate in the Activities, the Participant, for themselves and on behalf of their heirs, assigns, personal representatives, and next of kin, hereby releases, indemnifies, and holds harmless RFDC from any and all claims, demands, losses, and liability arising out of or related to any injury, disability, death or loss or damage to person or property, whether arising from the negligence of RFDC or otherwise, to the fullest extent permitted by law.
5. LEGAL RIGHTS: This Agreement means that the Participant cannot sue RFDC for any injury, loss, or damage sustained as a result of participating in the Activities. The Participant waives their right to sue RFDC for any reason.
By signing below, the Participant affirms that they are at least 18 years of age, or, if not, that they have secured below the signature of their parent or guardian as well as their own. The Participant has read and understood this Agreement, and they agree to be bound by its terms.
