LIABILITY WAIVER AND RELEASE AGREEMENT

Rhythmic Fusion LLC DBA: RF Dance Center

THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS – PLEASE READ CAREFULLY BEFORE SIGNING

This Release and Waiver of Liability (the "Agreement") is executed by the undersigned participant ("Participant") or the parent/guardian of a minor participant in favor of RF Dance Center, its officers, directors, employees, agents, independent contractors, and volunteers (collectively, "RFDC").

ACTIVITIES: The Participant (or the minor Participant) acknowledges that they will be participating in dance classes, workshops, and other related activities (collectively, the "Activities") at RFDC. These Activities may include, but are not limited to, ballet, Salsa, Bachata, Cumbia, and Banda dance.

RISK ACKNOWLEDGEMENT: The Participant (or the parent/guardian on behalf of the minor Participant) understands that the Activities involve inherent risks, including the risk of physical injury, disability, or death. The Participant willingly and voluntarily assumes full responsibility for all risks, both known and unknown, even if arising from the negligence of RFDC.

FACILITY INSPECTION: The Participant (or the parent/guardian on behalf of the minor Participant) acknowledges that RFDC has inspected the facility for safety. The Participant agrees to inspect the facilities and equipment to be used, and if they believe anything is unsafe, it is their responsibility to immediately notify the proper authority at RFDC and refuse to participate further.

RELEASE: In consideration of being allowed to participate in the Activities, the Participant (or the parent/guardian on behalf of the minor Participant), for themselves and on behalf of their heirs, assigns, personal representatives, and next of kin, hereby releases, indemnifies, and holds harmless RFDC from any and all claims, demands, losses, and liability arising out of or related to any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of RFDC or otherwise, to the fullest extent permitted by law.

LEGAL RIGHTS: This Agreement means that the Participant (or the parent/guardian on behalf of the minor Participant) cannot sue RFDC for any injury, loss, or damage sustained as a result of participating in the Activities. The Participant waives their right to sue RFDC for any reason.

By signing below, the Participant affirms that they are at least 18 years of age, or, if signing on behalf of a minor Participant, that they are the parent or legal guardian of the minor Participant and have the authority to sign this Agreement on their behalf. The Participant or parent/guardian has read and understood this Agreement and agrees to be bound by its terms.

Acknowledgment and Permission Clause for Recording and Use of Media
As the parent or legal guardian of the minor child, I hereby acknowledge and agree to the following terms and conditions regarding the participation of my child in dance classes at Rhythmic Fusion LLC DBA RF Dance Center ("RF Dance Center"):

Consent to Recordings:
I hereby give my full consent and permission for RF Dance Center, its instructors, staff, and authorized representatives to record, videotape, photograph, or otherwise capture the likeness and performance of my child during any and all dance classes, rehearsals, performances, or events conducted by or associated with RF Dance Center.
Use of Media:
I understand and agree that RF Dance Center may use, reproduce, publish, distribute, and display these recordings, photographs, or any other media on social media platforms, websites, promotional materials, flyers, and other marketing or training purposes without any financial compensation to me or my child.
Release of Liability:
I hereby release, discharge, and hold harmless RF Dance Center, its officers, directors, employees, instructors, and agents from any and all claims, demands, liabilities, and causes of action that may arise from the use of such recordings, photographs, or other media, including but not limited to claims for invasion of privacy or defamation.
Duration of Consent:
This consent and release shall remain in effect indefinitely unless revoked by me in writing, delivered to RF Dance Center. I understand that such revocation will not apply to any media that has already been used or published prior to the receipt of the written revocation.
By signing below, I acknowledge that I have read and understood the terms and conditions of this agreement and that I voluntarily agree to be bound by them.
