I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education, and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the instructor. I will continue to breathe smoothly. I assume full responsibility for any and all damages, which may incur through participation.
Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I will make the instructor aware of any medical conditions or physical limitations before class. If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against Esha Yoga and it's instructors.
I have read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my signature serves as a complete and unconditional release of all liability to the greatest extent allowed by law.

Dance Waiver form
In exchange for participation in Nrithyamanasa performing arts center activities such as, but not limited to, dance classes, rehearsals, private lessons, workshops, on and off premise dance showcase events, field trips, and fundraising, I agree to the following.
I recognize there are certain inherent physical risks associated with the above-described activities. I agree I am/my child is in proper physical condition and capable of participating in these activities and understand that Nrithyamanasa Performing arts center is not any way responsible for making such a determination.
I assume full responsibility for personal injury to me or my child, and further release and discharge Nrithyamanasa Performing arts center, its owners, employees, staff, guest artists, volunteers, agents, and representatives liable for my personal injury or any personal damage or loss, which may occur on Nrithyamanasa Performing arts center premises or offsite sponsored events, except in cases where Nrithyamanasa Performing arts center has been found to be solely negligent.
I fully understand that Nrithyamanasa Performing arts center staff members are not physicians or medical practitioners of any kind. With this in mind, I hereby release Nrithyamanasa Performing arts center to render first aid to me or my child in the event of an injury or illness, and if deemed necessary by Nrithyamanasa Performing arts center staff, to call 911 to seek emergency medical assistance.
Student Responsibilities
I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for me or my child and that Nrithyamanasa Performing arts center does not provide accident or health insurance for those participating in the above-mentioned activities.
I understand that it is my responsibility to bear financial restitution in incidents where my or my child’s inappropriate behavior caused physical injury to other persons, damage to personal property, or damage to facilities utilized by Nrithyamanasa Performing arts center.
I understand that there are no refunds, credits, make-up classes, or exchange for missed classes.
Photography/Video Release
I authorize and agree that Nrithyamanasa Performing arts center may take and use photographs, videos or likenesses of me or my child as needed for its record keeping, advertising, social media and/or public relations projects and that I have no rights to the same and will not be compensated for them.
I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND HAVE READ THE SAME PRIOR TO SIGNING, I AM OF LAWFUL AGE AND COMPETENT TO SIGN THIS AFFIRMATION.
If Student is under the age of 18, a parent or legal guardian must sign.
