I am at least 18 years of age or, if the new client is less than 18 years of age, I am a legal guardian of the new client. In consideration of this acceptance of my/their individual participation in class(es), I hereby waive any liability against Urban Fit Yoga Studio, Texas Thai Sports LLC, 5702 Feagan LLC,  and all other persons and entities, including but not limited to any facility owners/operators utilized during the class(es) and all sponsors, individuals, third parties, clients, and the like involved in any Classes, Workshops, Events, or otherwise connected or not connected with Urban Fit class(es) for any damages, physical, personal, or property, which may arise from my participation in the class(es). I agree that I am voluntarily participating in the class(es) and these activities and use of the facilities and premises and assumes risk of injury, illness, or death. Urban Fit Yoga, Texas Thai Sports LLC, 5702 Feagan LLC are not responsible for the safekeeping of my personal belongings while I attend the class(es).

I acknowledge that I have carefully read this Liability Release and fully understand that this is complete release of liability. I expressly agree to release and discharge all persons and entities, including but not limited to any facility owners/operators from any and all claims or causes of actions and I may otherwise have to bring a legal action against any of the foregoing for personal injury or property damage. To the extent that statute or case law does not prohibit release of negligence, this release also covers and includes negligence and any legal theory based upon negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be valid and/or unenforceable, then the remainder of this release from liability shall remain in full force and effect and offending provision or provisions severed here from.

I understand that video and photography recordings may be used in any class, workshop, or event that I attend and that I may be included in some of the video/videos, photo images.  I also understand and agree that I might be included in some of these videos and/or photos for marketing purposes. 

By agreeing to this release, I acknowledge that I understand its content and that this release cannot be modified orally. If the new client is under 18 years of age, as their legal guardian I consent to the above conditions.
