Please read the following statements carefully:

1. Attendant Risk and Discomforts:

There are risks associated with any form of activity. Classes may result in muscle and/or joint pain, pulled muscles, changes in blood pressure, light headedness, dizziness, delayed onset muscle soreness, tendonitis, and other discomforts. Training should be modified or postponed if symptoms exist or persist. 

Group classes are not suitable for everybody – some conditions require specialist modifications, understanding & training. NOT suitable if you are pregnant. Classes not recommended if you have a Heart condition/Angina; Epilepsy; High or Low Blood Pressure; Glaucoma; Thyroid Disorder; Cancer; Pneumonia or Pleurisy; Asthma; Joint injuries; Rheumatic Fever (Rheumatism); Fevers; Fainting or Migraine; Appendicitis; Stomach or Bowel Complaint; Diabetes; Infection of Kidneys; Haemophiliacs; Enclosed infections; Hypertension; Hypotension; Blood Coughing; Tuberculosis; Eating Disorder; Heart conditions/failure; Impaired coronary circulation or any other medical condition, illness or injury. If in any doubt use caution & consult your GP to get their approval before taking part.

2. Responsibilities of the Participant:

For your safety and benefit it is important you fully disclose your personal health history, any medications you are taking, and any symptoms you may be experiencing. Such symptoms  include back/neck/joint pain, irregular heart rhythm, tightness or pressure in your chest, unusual shortness of breath, light headedness, dizziness and the like. You should not exercise when you are ill or feel unwell.  

3. Benefits to be Expected:

You should notice various benefits as a result of regular and consistent participation. Yin Yan Classes typically result in numerous physical and mental benefits (including improved muscle strength, improved muscular endurance, increased flexibility, improved lung function, better relaxation and concentration).

4. Enquiries:

An important part of the informed consent process is providing you the opportunity to ask Yin Yan or your teacher about any aspect of the Classes. If you have any questions or concerns please email [email protected]

5. Privacy Policy

Any information gathered will be kept confidential to the extent provided by law. You will be encouraged to allow Yin Yan to share this information if it may help to diagnose or treat a current disease or reduce your risk of developing a more serious condition. No identifiable information will be released or revealed to any other party without your written consent. You may be asked, however, to allow certain information (from which your identity is removed) to be used for statistical analysis or research purposes.

If you are accidentally injured during a class, a Yin Yan Teacher or member of staff may be on hand to provide offer immediate first aid (if needed) but will be unable to provide full treatment. If injured, you will be responsible for seeking further investigation/treatment.

You acknowledge that you have read this form and Yin Yan’s T&Cs in their entirety and understand your responsibilities. You accept the risks, rules, and regulations set forth.
