I hereby release ONYX FIT/ONYX WHEELHOUSE and it's associates from any responsibility or liability due to my participation in all LIVE OR VIRTUAL Fitness classes. I am fully aware that I am participating in these sessions at my own risk and will not hold ONYX FIT/ONYX WHEELHOUSE and it's associates responsible in the event of my incurring any injury or exacerbating and previously existing conditions. I hereby authorize the staff at ONYX FIT/ONYX WHEELHOUSE to act for me according to their best judgement in an emergency requiring medical attention.
I fully intend to use common sense when participating in any fitness VIRTUAL OR LIVE classes and will be mindful of my own physical limitations and prior injuries so as not to sustain further damage. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any classes at ONYX FIT/ONYX WHEELHOUSE.
In checking the box below I agree that ONYX FIT/ONYX WHEELHOUSE and its associates are in no way responsible for the safekeeping of my personal belongings while I attend any live classes.
I understand that all classes LIVE AND VIRTUAL may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against ONYX FIT/ONYX WHEELHOUSE or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
**If applicable for family members under 18 years of age; I acknowledge and agree that I assume the risks associated with any and all activities and/or exercises in which my child participates.
I acknowledge and agree that no warranties or representations have been made to me or my child regarding the results I will achieve from any program. I understand that results are individual and may vary.
