Yoga Centric Liability Waiver

I have enrolled and I am participating in a health and fitness program of physical activity, which may include yoga postures, strength training, stretching, and breathing exercises with a yoga instructor at Yoga Centric.

I am aware that the temperature in any or all class formats at Yoga Centric range from 70F-105F degrees and that I am of sound mind and health to participate in any and all temperatures specified on the Yoga Centric class/workshop descriptions.

I hereby indemnify, Yoga Centric and the yoga instructor at Yoga Centric or any persons involved with Yoga Centric from any and all liability from injuries and damages resulting from participation in any activities or use of equipment or machinery involved in this fitness program.

I fully understand that the program may be physically strenuous and I choose to participate completely voluntarily.

I affirm that I am physically sound and suffering from no condition, impairment, or disease that would prevent my participation or use of equipment in this program.

I affirm that I have not had or been exposed to anyone with a communicable disease within the last 14 days

I acknowledge that I have had a physical examination and been given my physician’s approval to participate or have decided to participate in this program and use the equipment without the approval of my physician.

I understand that yoga is not a substitute for medical attention, examination, diagnosis or treatment of any medical condition.

Additionally, I understand that yoga is not recommended and is not safe under certain medical conditions. I will advise my yoga instructor about any significant health issue or condition, including injuries, and pregnancy before I begin this yoga program.

I understand that from time to time, I will receive “hands-on” assistance during these yoga sessions from a registered yoga instructor, in a manner that is safe and appropriate, and I am comfortable with this aspect of the studio's operations.

I understand that the Yoga Centric staff will maintain all aspects of this program as confidential, as it pertains to my client status and participation in classes.

I have read this agreement, fully understand its terms, understand that I have given up rights to hold Yoga Centric, associated person(s) or businesses, liable for injury, harm or negative impact to my physical or emotional well being.

I agree that I have acknowledged this waiver freely and without any inducement or assurance of any nature. I intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.

I understand that Yoga Centric participates in social media, public relations and advertising supported by in studio photography. Yoga Centric has my consent to use photographs that include my image for business related communications and sharing.

If this waiver is attached to a client profile who is under 18 years of age, I certify and agree that I am the parent or legal guardian of the client associated with this waiver and that I grant my permission for my child to participate in any classes at Yoga Centric that accommodate his/her age. In addition I am aware that any and all of the above statements apply to the minor child associated with this account.
